OVYM's Forms: Print a hard copy and use a pen, or download them and use the Adobe Reader to fill them in
Applications and Forms
All of our forms are in Adobe PDF format. Right click on the form you want and select Save target as... (or Save link as… for Chrome and Firefox) to save the PDF file to your computer. NOTE: Some browsers (i.e., Firefox) do not support fill-in forms and require that you download them to your computer, then open them in the Adobe Reader
You no longer need a pen to fill out and send OVYM forms! The forms have been updated so that you can fill them in using Adobe PDF Reader and save them to your local computer with the information you entered into the fields and email them. The Statistical Report even does the math for you. Right click on the file name and select Save As... to save it to a convenient folder on your computer. Then you fill in form's fields and save it with your information to print or email it to the recipient indicated on the form. PLEASE NOTE: To email the filled-in form directly from the Adobe Reader, select "Send file..." from the Adobe Reader's File menu. Read the Emailing a PDF document for the full set of instructions.
